Vertical control with centric bite block technique and 4X2

INTRODUCTION: The centric bite block (BBC) is a tool in orthopedic treatment that can be used for vertical control, both positive and negative, depending on the patient's diagnosis. Allowing posterior alveolar vertical control, in addition to maintaining joint stability during treatment. In patients with class II, it is an alternative to redirect growth in a positive direction.
OBJECTIVE: Restore occlusal plane and reduce or correct dental class II through the technique with BBC and 4X2 based on the stable condylar position (SCP).
CLINICAL CASE: A 9-year-old male patient with late mixed dentition was presented to the ENES clinic, UNAM. He was diagnosed as mesofacial biotype, dental class II in PCE, tooth 22 in palatal malposition. No cross-sectional discrepancy. Positive vertical control is performed with BBC with a height of 4mm and anterior alveolar control through the 4X2 technique with Roth 0.022 self-ligating Memory-type brackets.
In 8 months, a decrease in facial axis from 91.1 to 86.3 degrees and convexity from 2.6mm to 2mm was achieved, confirming positive mandibular rotation.
CONCLUSIONS: As pediatric dentists, we are responsible for carrying out a thorough examination for the diagnosis of malocclusions, taking advantage of the stage of growth and development of the individual, to later plan and carry out orthopedic treatments with the aim of intercepting the malocclusion that affects the dentofacial complex. This will allow to prevent the increase of its severity and will promote the harmony and functionality of the stomagnatic system.
