Introduction: supernumeraries are additional teeth that develop more frequently in the maxilla, with a greater predisposition in the male gender, their etiology is unknown, a combination of environmental and genetic factors has been proposed to explain the appearance of these, they occur with a prevalence of 0.1-3.8% in the permanent dentition. They are classified according to their location, morphology, eruption and root development. Extraction of supernumeraries is indicated when they cause dental retention, spaces, crowding, radicular cysts, root resorption, delayed eruption or ectopic eruption. The objective is to show a clinical case of the upper central incisor retention treatment due to unilateral mesiodens included group 1 in a pediatric patient. Case presentation: 10.8-year-old male patient with no pathological history. Oral examination revealed root remnants of the right temporal central incisor and posterior crossbite. Radiographically, mesiodens was found including group 1 and retained permanent central incisor. As a treatment plan, exodontics of the root residue and the supernumerary was performed. It was decided to wait for the spontaneous eruption of 11 and he was referred for interceptive orthodontics. With postoperative follow-up without alterations. Interceptive orthodontic treatment was performed one month after the surgical intervention, with a Hyrax appliance in order to expand the maxilla transversely, allowing the correction of the posterior crossbite and providing space for the eruption of the dental organ 11. Conclusions: the timely diagnosis and treatment of dental retention caused by supernumeraries is important.
