Introduction: The preservation of primary teeth until their exfoliation is important since they are considered the best natural maintainers of space and guide the correct eruption of permanent teeth. Non-instrumented endodontic treatment or LSRT, is a simple technique that avoids anatomical complications that may occur at the time of instrumented pulpectomies. This technique uses a combination of 3 antibiotics for sterilization of root canals and healing of periradicular tissues. Objective: Report the pulp treatment of a necrotic tooth with the LSRT technique. Case presentation: A 7-year-old girl patient comes to the service presenting a lesion in the gum, clinically a deep carious lesion is observed in tooth 8.4, and a vestibular fistula and grade 2 mobility. Radiographically a radiolucent image compatible with caries lesion with pulp involvement and apical radiolucent image compatible with dentoalveolar abscess. The diagnosis was pulp necrosis and dentoalveolar abscess. LSRT treatment was performed followed by metallic crown. Conclusion: The LSRT technique presents a high success rate and helps in the preservation of the tooth in the mouth, the results of this case are consistent with the findings in the literature. Carrying out clinical and radiographic controls will help us to obtain a good treatment.
