Pre-surgical orthopedics in neonate with bilateral cleft lip and palate.

Introduction: Congenital malformations are structural defects present at birth. Cleft lip and palate are anomalies that are commonly associated and constitute the most frequent malformations of the head and neck, cleft lip is the facial defect that involves incomplete closure of the lip, presenting unilaterally, bilaterally or medially. Objective: To describe the benefits of early treatment with pre-surgical orthopedics in a patient with severe incomplete bilateral cleft lip-alveolus. Description of the case: Male newborn of 4 days of extrauterine life, with severe bilateral incomplete cleft lip-alveolus palatine, (17mm wide) who goes to the Comprehensive Pediatric Dentistry Clinic of the Autonomous University of Zacatecas for care. We worked in a multidisciplinary way, with a Pediatrician from the general hospital, followed by a Pediatric Dentist and during the treatment with a Maxillofacial Surgeon. Due to the severity of the fissure, treatment with an obturator plate is started, which can be placed from the first day of birth, it is modified monthly, for 4 months to facilitate feeding, stimulate the decrease of the fissure and improve the style. of life. Conclusion: The important thing in the treatment of this pathology is the multidisciplinary management by a group of specialists deeply internalized in the subject with great interaction in decision making. The permanent concern to achieve better results has led to the current situation of early management with pre-surgical orthopedics and primary correction of the lip, nose and gums with early closure of the palate.
