Surgical technique of bilateral multiple tubercular mesiodens erupted vertically Group V

Abstract

Introduction: Mesiodens is the presence of supernumeraries located in the midline area of the maxilla. Its frequency in mixed dentition is 0.6-1.7%. They are classified according to their location in mesiodens located between upper central incisors; single or multiple, unilateral or bilateral, erupted or impacted, vertical, horizontal or inverted. According to their morphology in tubercular: larger, with one or more accessory cusps, commonly in the palatal area at the level of maxillary central incisors. According to their root development five groups are determined and corresponds to Group V: complete root formation. The surgical procedure for supernumerary teeth is exodontia. Objective: To present the technique of surgical exodontia of Group V vertical erupted bilateral multiple tubercular mesiodens. Case presentation: 10.5 years old male patient. Intraoral examination showed mixed dentition and two erupted supernumeraries in the upper anterior area in the midline, tubercular in shape. Radiographically there was an absence of carious lesions, presence of two supernumeraries with complete root formation. The diagnosis was bilateral multiple tubercular mesiodens erupted vertically Group V. The surgical technique was performed in collaboration with the Oral Surgery department, under local anesthesia with supraperiosteal and palatal block, luxation and exodontia of both mesiodens, then a hemostatic sponge and a surgical stitch were placed. The clinical and radiographic follow-up after six months of alveolar healing was adequate. Conclusions: The surgical technique for supernumerary teeth is indicated in cases of severe dental crowding, displacement of adjacent teeth and alteration of esthetics.
