Introduction: Infective endocarditis prophylaxis aims to prevent the development of this disease in patients at risk when they undergo procedures that cause bacteremia. This antimicrobial prophylaxis is used in patients with high-risk heart disease, such as complex cyanotic heart disease dsvd. Prevalence of 0.8% live births. 2:1 higher incidence in men. It is diagnosed by echocardiogram, tomography or magnetic resonance that will indicate its type, complexity and severity. Objective: To know the management of patients with complex cyanotic heart disease dsvd. Clinical case: A 5-year-old female attends the consultation for a general check-up. Systemically, he presented complex cyanotic heart disease dsvd. Clinically, carious lesions are observed, except for the lower anterior and lower right sectors. Procedures were performed for 6 appointments with prior antimicrobial prophylaxis (Amoxicillin 50 mg/kg/d or 1 hour prior), and supported by the use of nasal prongs for oxygen administration and 2 caries control appointments. Results: The procedures were performed without complications. Conclusions: The role of the pediatric dentist is to find out about the patient's systemic status in order to make a personalized and safe approach to it. Antimicrobial prophylaxis treatment is essential for patients with this condition.
