Non-instrumented pulpectomy with CTZ in deciduous molar, a 9 month clinical and radiographic follow-up

Abstract

Introduction Caries remains a global problem. In most cases, children receive care too late, resulting in an association with pulp pathologies. The main goal of a pulpectomy is to preserve the tooth in the oral cavity until it completes its exfoliation. The use of CTZ paste is an alternative for the treatment of non-instrumentalized pulpectomies in temporary molars with pulp necrosis. Objective Management of pulp necrosis in a deciduous molar using the non-instrumented pulpectomy technique to return health to the dental organ and see its evolution through clinical and radiographic follow-up. Case Report A 5-year-old female reported to the Centro de Atención Medica Integral of the University of Guadalajara with an odontogenic facial cellulitis associated with dental organ 75. On intraoral clinical examination, an extensive caries lesion was observed involving the pulp chamber and prested radiographically lesion in furca. ICDAS code 5 was diagnosed. Prior to treatment, Clindamycin was prescribed and the non-instrumented pulpectomy treatment was performed by filling the cavity with CTZ paste and restoring with GC Fuji IX GP. An elastic separator of 4 mm Ormco was placed between the molars and a chrome steel crown was adapted with the Hall technique and cemented with Ketac Cem Easy Mix. Conclusions At 9 months the patient is asymptomatic and has no infectious recurrence. Bone apposition is observed radiographically in radiolucent areas indicating short- and medium-term success of treatment.
