Ectopic eruption of the first permanent molar (MEE) is a positional anomaly due to an alteration in the eruption pathway in the mesial direction with respect to its normal path, with a prevalence of 2 to 6%. It can cause premature loss of the primary second molar with a decrease in the arch perimeter and other alterations in the occlusion. The aim of this case report is to describe mesial ectopic eruption of the first permanent molar in 3 pediatric patients with different characteristics including reversible and complex cases in healthy and medically compromised patients. Case 1 is a 7 year old male patient who presented 16 and 26 irreversible grade III and IV, 55 MEE was successfully treated with copper separator and 65 was extraction and space maintainer due to pain and mobility. Case 2, a 7 year old female patient who presented with 16 and 26 grade IV irreversible EEM, underwent extraction and space maintainer as the first phase. Case 3 was a 7 year old female patient with Job's Syndrome who presented 16 and 26 MEE and dental agenesis, in whom space loss was assumed. Conclusion: The diagnosis and timely individualized treatment of MEE should be carried out taking into account the patient's health, collaboration and malocclusion characteristics, as well as the degree of resorption and symptomatology of the primary second molar and the amount of inclination of the permanent molar.
