Introduction: Alveolysis is characterized by the root exposure of the apical portion (apical fenestration) and/or vestibular (total or partial dehiscence) of a traumatized, necrotic tooth, which suffered several episodes of reinfection, dentoalveolar abscess, which causes resorption of the vestibular bone table with or without involvement of the alveolar-basal bone. Objective: Describe the surgical approach to a temporary tooth with alveolysis. Case Report: A female patient aged 6 years and 4 months presents with apical fenestration type alveolysis in tooth 61. The clinical and radiographic examination is carried out and the treatment plan is decided to extract the tooth, subsequent control and eruption of the tooth. the permanent piece, thus alleviating the discomfort reported by the patient. Conclusions: It is evident that alveolysis is an entity that can be known as fenestration, it is divided into: fenestration and total or partial dehiscence. It is essential to make a timely diagnosis and treatment, thus eliminating the focus of infection and promoting the eruption of the successor tooth.
