Functional orthopedics with indirect flat planes: a clinical case report.

Abstract

Case report: A 9-year-old female, mother reported frequent and intense snoring while sleeping. Radiographic studies revealed a narrowly patent airway. Otorhinolaryngology consultation indicated surgical prognosis for nasal septum correction. Extraorally, mandibular deviation upon closure was found, with no history or presence of joint noises or pain, and an inadequate masticatory pattern with a predilection toward the left side. Class I indirect flat planes were placed. Monthly follow-up was performed for adjustment of vestibular arches and frontal springs. Activation of upper and lower expander screws 1/4 turn per week was indicated. Results: improved arch conformation, improved dental alignment, improvement in mandibular deviation, bilateral masticatory pattern. The mother reported a decrease in the frequency and intensity of snoring, the patient mentioned greater energy upon waking up. A radiographic study showed an increase in airway patency. Conclusions: Early and transdisciplinary detection and intervention in respiratory and muscular disorders is essential to prevent permanent structural problems and ensure harmonious orofacial development.
