Correction of anterior open bite caused by tongue thrusting with fixed tongue crib: A clinical case

Introduction: Among the most common malocclusions is anterior open bite, which accounts for between 25% and 38% of cases. This is defined as the absence of vertical contact between the anterior teeth and is associated with persistent oral habits, such as tongue thrusting — a repeated pushing of the tongue against the front teeth that disrupts the orofacial muscular balance and contributes to the development of open bite.
Case Report: A 6-year-old female patient attended the Comprehensive Medical Care Center of the University of Guadalajara. The initial medical history revealed no relevant data or systemic conditions. Intraoral examination showed the absence of teeth #51 and #61, as well as the presence of an anterior open bite. Tests were conducted to determine the etiology of the open bite, and tongue interposition was found. The chosen treatment was the placement of a fixed tongue crib to prevent the repeated tongue thrusting. At a subsequent appointment, the tongue crib was cemented. After 3 months, a significant reduction in the open bite was observed, as well as the eruption of tooth #21.
Conclusions: The proper management of anterior open bite requires understanding the etiological factor; early intervention is key to achieving correction and preventing future complications. The tongue crib is an effective option to correct this habit and achieve stable results.
