Aesthetic-functional management in incisors with severe HMI in permanent dentition.

Introduction: Molar incisor hypomineralization (MIH) is a developmental defect of the enamel that primarily affects first molars and permanent incisors. It is characterized by defined or diffuse opacities, dental sensitivity, and accelerated loss of structure, compromising dental function and aesthetics. The objective is to find an alternative for the treatment of severe hypomineralization in the anterior sector in young permanent dentition. Case Report: A 9-year-old female presents for consultation due to aesthetic alterations in the anterior sector. Clinically, the dental organs 11, 12, 21, and 22 are observed with severe molar-incisor hypomineralization. It is decided to perform direct restoration under absolute isolation protocol. The clinical protocol included micro-abrasion, etching, adhesion, and placement of the Beautifill II giomer. Follow-up showed satisfactory functional and aesthetic results. Conclusions: This bioactive material represents an effective alternative due to its sustained release of fluoride, remineralizing effect, low biofilm adhesion, and simple clinical application without the need for specialized instruments.
