Use of distal slipper due to premature loss of second molar: Case report

Introduction: The primary dentition should maintain its integrity until the moment of tooth replacement, it stimulates the formation of alveolar bone, accompanies craniofacial development and growth and prevents the establishment of malocclusions. The premature loss of the second temporary molar predisposes the mesialization or inclination of the first permanent molar, with it the presence of crowding, ectopic eruptions, among other alterations. When the first permanent molar has not erupted, therapeutic options to maintain space for a second primary molar are limited. Objective: To spread the use of the distal slipper maintainer, despite its perception as a sensitive and complex technique, and which is not indicated for all types of patients. Inform its advantages and disadvantages. Case report: Female patient, 4 years 4 months, with early childhood caries disease. After intervening on his oral biological risk factors, tooth 7.5 was extracted and a distal slipper maintainer was installed. The technique used and its follow-up 6 months after treatment are described. Conclusion: Distal slipper maintainer requires careful patient selection and planning. Through distal extension, it is capable of guiding and promoting the correct eruption of the first permanent molar, being highly cost-effective in preventing future complications that require more complex and expensive orthodontic interventions.
