Early orthopedic management of Angle malocclusion class III: case report

Introduction: Angle malocclusion class III is a skeletal disorder that compromises the harmonious growth of the jaws, facial aesthetics and masticatory function. Its early diagnosis allows intervention during active phases of development, which can modify the growth pattern. Case report: 5-year-old female patient diagnosed with skeletal, dental and functional class III malocclusion, characterized by presenting a concave profile with decreased lower facial third, maxillary micrognatism and retrognatism, mandibular prognathism, anterior cross bite, 100% dental midline deviation and vertical overbite. In March 2022 orthopedic treatment was initiated using a Petit mask anchored to the upper Quad Helix with bands for expansion and maxillary traction, and lower Hawley plate with posterior bite plane. At the age of seven months, there was evidence of uncrossing of the previous bite and a decrease in the vertical overbite to 36%; a positive change in the facial profile and proportionality between the facial thirds was observed during the year. Conclusions: Early treatment of class III malocclusions based on maxillary expansion and anterior traction may be effective in redirecting facial growth. In the reported case, the vertical overbite was reduced and facial proportionality was restored, reducing the need for subsequent invasive treatments and demonstrating stability in the advances obtained.
